AI for preventing heart disease
ScriptChain Health is an AI-powered healthcare platform designed to reduce hospital readmissions for heart disease by providing personalized nutrition and exercise plans to patients.
The company has developed and patented an AI system that identifies patients who are at high risk for conditions like heart failure, high blood pressure, irregular heartbeat (atrial fibrillation), and coronary artery disease by integrating with electronic health records. Once identified, patients are provided customized dietary and fitness plans to help prevent their conditions from worsening. These plans are generated using an LLM trained on trusted sources like medical journals, clinical studies, nutrition textbooks, and diverse population health data.

One of ScriptChain’s unique features is that it automatically generates CPT (Current Procedural Terminology) and HCPCS (Healthcare Common Procedure Coding System) billing codes, which are how hospitals and clinics report services to insurance companies to get paid. This helps ensure healthcare providers can be reimbursed for offering preventive care, including nutrition and exercise planning.
Together, these are part of what ScriptChain calls its "Food and Exercise as Medicine" program. They not only open up new revenue streams for hospitals but help them cut costs. Why? If a patient ends up back in the hospital within 30 days of being discharged, the hospital does not get reimbursed by insurance for that second visit. Thus, by helping patients stay healthy and avoid readmission, ScriptChain improves both patient outcomes and the hospital's financial performance.

After the death of his grandmother from coronary heart failure, ScriptChain CEO Moh Noori began questioning the healthcare system. He grew frustrated with its heavy reliance on medication, which often ignores the lifestyle changes needed to address the root causes of disease, and believed there had to be a smarter way to predict and prevent illness. While studying information systems and business management at San Francisco State University, and later while working as an account manager in the medical device industry, he gained firsthand exposure to the gaps in healthcare. During his MBA studies, the solution began to take shape. After learning that 97% of EHR data goes unused, he began exploring how machine learning and neural networks could uncover hidden patterns and improve patient outcomes. Motivated by his grandmother’s story, he founded ScriptChain to help healthcare systems move from reactive treatment to proactive prevention.

ScriptChain Health is particularly compelling because of its solution’s overlap with current objectives in federal and state policy.
Recently, there have been several major federal initiatives and legislative actions focused on integrating nutrition, exercise, and advanced technology into healthcare delivery. For example, the National Food As Medicine Program Act of 2024 established grant programs and technical assistance to help healthcare systems implement medically supportive food interventions, with the U.S. Department of Health and Human Services tasked to issue guidance and report on outcomes.
States are also taking action. At least 10 have received approval from the Centers for Medicare & Medicaid Services for Medicaid waivers to fund food as medicine interventions for eligible populations, and several more are pursuing similar policies. Legislation in states like California, Illinois, New Jersey, and New York aims to make medically tailored meals and nutrition services a permanent, covered benefit under state-run health plans. Additionally, organizations such as the National Association of State Departments of Agriculture have formally endorsed the expansion of food as medicine programs.
Collectively, these policy shifts are creating a supportive environment for platforms that provide nutrition and exercise interventions. ScriptChain is perfectly aligned with these trends, given its combination of innovative non-pharmaceutical interventions and reimbursement support.
